Health requirement and OSHC for the subclass 500 student visa.

Quick answer

A subclass 500 visa requires OSHC maintained for the whole visa period (condition 8501) plus, in many cases, a health examination. Gaps in cover are a common, avoidable refusal and cancellation trigger.

  1. 01Current public rule: Home Affairs states that condition 8501 requires the visa holder to maintain adequate health insurance (OSHC) for the duration of their stay in Australia. Health examinations are required for stays over 6 months.
  2. 02Controlling source: Department of Home Affairs (health requirement, OSHC, condition 8501), privatehealth.gov.au for OSHC provider information.
  3. 03Main risk: OSHC start or end dates not matching the visa period, dependants not covered, or health examination results expiring before the visa decision.

The health requirement for the subclass 500 visa

Home Affairs requires all subclass 500 student visa applicants to meet the health requirement. This has two components. First, you must have adequate health insurance (Overseas Student Health Cover, or OSHC) for the entire period of your intended stay in Australia, as required by condition 8501. Second, you must undergo a health examination if you plan to stay in Australia for more than 6 months, if you are applying for a visa that specifically requires health assessment, or if you have a medical condition that may require significant treatment or care during your stay.

The health examination is arranged through Home Affairs' panel physician system. You first receive a HAP ID (Health Assessment Pointer) reference number from Home Affairs, which you use to book an appointment with an approved panel physician in your country. The physician conducts a medical examination (including a physical check, chest x-ray, and blood tests) and submits the results electronically to Home Affairs. The results are typically valid for 12 months from the date of the examination.

If you do not meet the health requirement β€” for example, if you have a condition that would impose significant cost on the Australian healthcare system β€” Home Affairs may refuse the visa. However, some health conditions can be managed with a health waiver if you have strong compelling circumstances and the cost is below the threshold set by Home Affairs. The waiver is not automatic and requires a specific application.

Who this guide is for

Prospective subclass 500 visa applicants who need to understand the health examination and OSHC requirements before applying. Current student visa holders who need to renew their OSHC or update their health examination before applying for a subsequent visa. Dependants of student visa holders who need their own health examinations and OSHC cover.

This guide covers the health requirement specifically. It does not restate the full document checklist for the subclass 500 β€” for the complete checklist including CoE, GS statement, financial capacity, and English evidence, use the subclass 500 document checklist.

Documents and evidence to prepare

OSHC policy documentation. The policy certificate from an approved OSHC provider showing coverage dates that span your entire intended stay. The policy must cover the main applicant and all dependants included in the visa application. Check the policy start date β€” it should be the date you expect to arrive in Australia (or your current visa expiry date if applying onshore). The policy end date must be after the expected visa end date.

HAP ID and health examination receipt. The HAP ID letter from Home Affairs and the appointment confirmation with the panel physician. After the examination, the panel physician provides a receipt or referral letter β€” keep this as evidence that the examination was completed. The results are sent directly to Home Affairs electronically.

Medical evidence if you have a pre-existing condition. If you have a pre-existing medical condition, provide specialist reports, treatment plans, and evidence that the condition is stable and manageable. This helps Home Affairs assess whether the condition would impose significant cost on the Australian healthcare system. A well-documented condition with a positive prognosis is less likely to trigger a health refusal.

Deadline and risk triggers

The health examination results are valid for 12 months. If your visa application takes longer than 12 months to process, or if you defer your studies and reapply after 12 months, you may need a new health examination. Plan your examination timing so that it will still be valid when the visa decision is expected. The OSHC policy must be purchased before the visa is granted β€” you cannot promise to buy it later.

The main risk is a gap in OSHC cover. Condition 8501 requires continuous cover for the entire visa period. A gap of even one day is a breach. If you change OSHC providers, ensure the new policy starts on the day the old policy ends. If you are in Australia and your OSHC lapses, Home Affairs can cancel your visa for breach of condition 8501.

A second risk is not covering dependants. If your OSHC policy only covers the main applicant but your visa application includes dependants, the application may be refused. Purchase a family OSHC policy that covers all family members included in the application. If dependants join you later, they need their own OSHC from the date they arrive.

Worked example: OSHC gap prevention

Elena, a 23-year-old student from Brazil, was granted a subclass 500 visa valid from March 2024 to March 2027. She purchased a 3-year OSHC policy from an approved provider. In September 2025, she extended her studies to a second course, which required a visa extension. When applying for the subsequent visa, Elena realised that her OSHC policy expired in March 2027 but the new visa would extend to March 2028.

Elena contacted her OSHC provider and purchased a one-year extension to cover the additional visa period before submitting the new visa application. She submitted the new OSHC certificate with the application, demonstrating continuous cover from March 2024 to March 2028. The subsequent visa was granted without a health-related request for further information.

Choosing an OSHC provider

Home Affairs requires OSHC from an approved provider. Major providers include Australian Health Management, BUPA, Medibank, and Allianz Care Australia. The standard features of each OSHC product vary between providers, so read the product disclosure statement carefully before purchasing. Compare policies based on: coverage for the full visa period, level of cover (single vs family), hospital and medical benefits, pharmacy coverage, ambulance cover, and the provider's network of doctors and hospitals. Some providers offer student-specific policies with additional mental health services.

You can purchase OSHC directly from the provider or through your education provider. Some universities offer OSHC as part of the enrolment package. If you purchase through your provider, confirm the policy start and end dates match your visa period. If purchasing independently, upload the policy certificate to ImmiAccount at the time of visa application.

Disability services and additional support

If you have a disability, chronic health condition, or mental health condition that affects your studies, register with your university's disability services office as early as possible β€” ideally before you arrive in Australia. Disability services can arrange academic accommodations such as extended exam time, note-taking support, accessible housing, and assistive technology. These services are separate from your OSHC cover and are provided by the university at no additional cost.

Your OSHC policy may not cover all the healthcare services you need for ongoing management of a disability or chronic condition. Check whether your policy covers allied health services such as physiotherapy, occupational therapy, or psychology sessions. If your condition requires regular specialist appointments, confirm the waiting periods and annual limits under your specific OSHC policy before relying on it for ongoing care. For students with mental health needs, most universities offer free counseling services through their student wellbeing centres, which can supplement the mental health resources available through your OSHC provider.

Health examinations for dependants

If your dependants (partner or children) are included in your visa application, they may also need health examinations. The requirement depends on their age, the length of stay, and the country they are applying from. Dependants over 11 years old typically require a chest x-ray. All dependants must have OSHC cover that starts from their arrival date in Australia. If dependants join you after the visa is granted, they need separate health examinations and OSHC from the date of arrival.

Condition 8501 compliance and penalties

Condition 8501 on the subclass 500 visa requires you to maintain adequate health insurance for the entire duration of your stay in Australia. A gap in OSHC cover is a breach of this condition. If Home Affairs discovers a gap, they can issue a Notice of Intention to Consider Cancellation. Common reasons for gaps include: forgetting to renew before the policy expires, switching providers without overlapping cover, or assuming OSHC is automatically renewed through your education provider when it is not.

Keep evidence of your OSHC cover including policy certificates, renewal notices, and payment receipts throughout your stay. If you change providers, obtain a letter from the old provider confirming the policy end date and a certificate from the new provider confirming the policy start date. If Home Affairs requests evidence of health insurance during a compliance check, you must produce these documents promptly.

Health examinations outside Australia

If you are applying for a subclass 500 visa from outside Australia, you must complete your health examination in your home country or country of residence with an approved panel physician. Home Affairs maintains a list of panel physicians by country on their website. After the examination, the panel physician submits the results electronically to Home Affairs. You do not need to submit the results yourself β€” but keep the receipt and HAP ID letter as evidence that the examination was completed.

Health examination results are valid for 12 months from the date of the examination. If your visa application takes longer than 12 months to process, or if you defer your studies and reapply after 12 months, you may need a new examination. Plan the timing of your health examination so it will still be valid when the visa decision is expected. An examination done too early risks expiring before the visa is granted.

What to do if you are refused on health grounds

If Home Affairs refuses your visa application on health grounds, the refusal will cite the specific health criterion you did not meet. The most common refusal reason is that your medical condition would impose significant cost on the Australian healthcare community, exceeding the thresholds set by the Migration Regulations. You may be eligible for a health waiver if you can demonstrate compelling circumstances β€” such as a close family connection to Australia, a job offer in a skilled occupation, or that the condition is unlikely to require significant treatment during your stay. A health waiver application should include specialist reports, treatment plans, cost estimates, and a letter explaining your circumstances. Seek advice from a registered migration agent before applying for the waiver, as the requirements are strict and the outcome is discretionary.

OSHC claims and excess payments

When you use your OSHC policy to access medical services, your provider will have specific arrangements for claims. Some providers offer direct billing arrangements with certain doctors and hospitals, meaning you present your OSHC membership card and the provider bills the insurer directly. For other services, you pay the medical fee upfront and claim a refund from the insurer. Most OSHC policies have an excess payment per incident or per year, and you should check the excess amount in your policy before accessing services.

Your OSHC policy covers the Medicare Benefits Schedule fee for medical services, but not the full amount a doctor may charge. If your doctor charges above the MBS fee, you may need to pay the gap. Some providers offer gap-cover or no-gap arrangements with certain doctors. Before booking an appointment, ask the doctor if they have a direct billing arrangement with your OSHC insurer and check whether you will have a gap payment. Keep all receipts and claim forms as you may need to submit them to your insurer for reimbursement.

Frequently asked questions

What is OSHC and do I need it for my student visa?+

OSHC (Overseas Student Health Cover) is health insurance for international students in Australia. Condition 8501 on the subclass 500 visa requires you to maintain OSHC for the entire duration of your stay in Australia. You must purchase OSHC before the visa is granted and maintain it without gaps for the whole visa period. Study Australia states that OSHC covers medical and hospital services but does not cover dental, optical, or physiotherapy.

When do I need a health examination?+

Home Affairs requires a health examination if you plan to stay in Australia for more than 6 months, if you are applying for a visa that requires a health assessment, or if you have a medical condition that may require treatment. The examination includes a medical check, chest x-ray, and blood tests. You need a HAP ID (Health Assessment Pointer) from Home Affairs before you can book the examination with an approved panel physician.

How much OSHC cover do I need?+

The OSHC policy must cover the entire period from your expected arrival date in Australia to the end of your student visa. If you are already in Australia, the cover should start from the date your current visa expires or the date your student visa is granted. Home Affairs states that a single OSHC policy should cover the main applicant and all family members included in the application. You can purchase OSHC from an approved provider.

Does OSHC cover my dependants?+

Yes, if you purchase a family OSHC policy. If your dependants are included in your visa application, your OSHC policy must cover them for the same period. If dependants join you later, they must have their own OSHC cover from the date they arrive. Check with your OSHC provider whether their policy covers dependants who arrive mid-policy.

What happens if my OSHC lapses?+

A gap in OSHC cover is a breach of condition 8501. If Home Affairs becomes aware of a gap, they may issue a Notice of Intention to Consider Cancellation (NOICC) under section 116 of the Migration Act. To avoid this, renew your OSHC before it expires and keep proof of continuous cover. If you change providers, ensure there is no gap between the old and new policies.

Can my OSHC policy be refunded if I leave Australia early?+

Yes, most OSHC providers offer a refund for the unused portion of your policy if you leave Australia permanently before the policy end date. Check the terms of your specific policy. You will need evidence that you have departed Australia (boarding pass, departure stamp, or movement record from Home Affairs).

What if I have a medical condition that requires treatment in Australia?+

You must declare any pre-existing medical conditions on the visa application. Home Affairs may require additional health examinations or a Medical Treatment visa subclass 602 if the condition requires specific care. OSHC covers treatment for pre-existing conditions but may have waiting periods and exclusions. Read your OSHC policy details carefully before purchasing.
